		Driveway Permit or Waiver Letter from Livingston County Road Commission
		Record closest existing addresses on survey
Subdivisions or Condominiums	This form is designed for  multi-unit developments.  All addresses for the development must be applied for & paid for at one time.  Visit our FAQ page for more details.	Final development site plan
		Letter of approval for all street names from Livingston County Road Commission
		Parcel ID numbers for all parent parcels included in development
		Legal description for entire development
Electric Meter	Similar to the single address form, but only  information listed at right is required.  Visit our FAQ page for more details.	Current parcel number
		Survey or site plan showing location of meter(s)
		Record closest existing addresses on survey
The fee for each address is $20.  Payment is accepted in cash or check.
